Enclosed is a quick examination packet that the partners would like for you to work through. The examination is split into two parts: Practical and Question/Answer.

Part 1: Practical Exam

  1. The practical exam consists of a Questionnaire. You are going to be provided with: (1) Crash Report; (2) PNC Narrative. 
  2. Please fill out the Questionnaire provided to you to the best of your ability.

Part 2: Question/Answer Exam

  1. You are also being supplied with a Case Manager Manual.
  2. For some questions, there will only be 1 correct answer.
  3. Please use your materials to answer all the questions, if needed.
  4. If you choose to step out during your exam, your exam will be considered completed.
